Immunization Exemptions

In Ontario, children must be up to date on their immunizations or have a valid exemption on file with public health to attend school or licensed childcare. The Immunization of School Pupils Act outlines which vaccines are needed for children in school and the Child Care and Early Years Act outlines which vaccines are needed for children in licensed childcare.

Immunization Exemptions for Children in School (K-12)

If you are choosing not to immunize your school-age child, understand the risks and responsibilities.

To exempt children attending primary or secondary school or who are registered for Junior Kindergarten in September of the same year, parents or legal guardians will need to:

  1. Complete a mandatory education session facilitated by a Public Health Nurse. This can be arranged by calling 705-743-1000, ext. 242. A Vaccine Education Certificate will be issued upon completion.
  2. Complete a Statement of Conscience or Religious Belief form (affidavit)that must be witnessed by a commissioner for taking affidavits (notarized). *An education session (step 1) will need to be completed prior to the affidavit signing, as there is a requirement to show the Vaccine Education Certificate to the commissioner.
  3. Submit the original unaltered Statement of Conscious or Religious Belief form to Peterborough Public Health by mail or drop-off:

    185 King St, 3rd floor, Peterborough, Ontario K9J 2R8
    Affidavit forms that have been altered e.g., additions or information crossed-out, are not accepted.
  4. Keep a copy for your records.

*Affidavits under the Immunization of School Pupils Act will not be accepted for non-school age children below 3 years of age. Parents of non-school age children attending childcare must complete the affidavit outlined in the licensed childcare exemption section below.

Medical Exemptions

If your child is unable to be immunized due to medical reasons, you will need to:

  • Request your health care provider complete the Statement of Medical Exemption form.
  • Submit the completed form to Peterborough Public Health by:

  • Keep a copy for your records.

Immunization Exemptions for Children in Licensed Child Care

If you are choosing not to immunize your child attending childcare for religious or conscientious reasons, you will need to complete a Statement of Conscience or Religious Belief for Childcare form and have it witnessed by a commissioner for taking affidavits (notarized) and submit to your childcare centre and Peterborough Public Health. *Please note an education session is not required and this form will expire when your child begins school.

If your child attending childcare cannot be vaccinated for medical reasons, please see medical exemption section above.
