Immunization for Children

Immunizations Required for Child Care Centres

Attendees of Child Care Centres must be immunized based on the recommendations of the Medical Officer of Health according to the Child Care and Early Years Act.

Children in child care centres whose immunization records are incomplete may be suspended from child care if this information is not provided. Parents/guardians are asked to provide immunization information to Peterborough Public Health to avoid suspension.

Medical Exemptions

If your child is unable to be immunized due to medical conditions, your healthcare provider can complete a medical exemption form. A doctor’s note is not accepted.

Philosophical Exemptions

If You Choose Not to Vaccinate Your Child, understand the risks and responsibilities. Children in schools and in child care centres require a valid exemption on file.

Please note: Philosophical exemptions need to be notorized.

If you have completed an exemption for child care in the past, you will need to complete a new exemption form by September 2017. If your child has transitioned to school, you will need to complete another exemption form under the Immunization of School Pupils Act.

Forms are also available in-person at Peterborough Public Health or can be mailed to you upon your request.
